    If you are uploading teeth impression photos and you ordered a Partial or a Flipper, please check the boxes below of the teeth that you want to be added to the teeth device.

    You only need to select the teeth you want to be added and the shade listed below one time per order and not every time you upload photos.

    Please select the Shade (or Color) number of the Tooth listed below that best matches your existing teeth.


    Add image file using .jpg, .png, .tif, or .gif (Click on X only if you want to remove the File)

    Upload a maximum of 2 photos due to the 5 megabyte file size limit.
